چکیده
Background In non-small cell lung cancer (NSCLC), response to checkpoint blockade therapy (CBT) is associated with tumor-infiltrating CD8+ T cells, but not all cell-infiltrated tumors respond CBT. The subgroup of CBT-resistant has been clinically described as containing ”non-functional” responses. Mechanisms governing the generation non-functional responses remain poorly understood, and treatment options for this are limited. Methods We utilized a transplantable, syngeneic murine NSCLC line derived from an autochthonous driven by Kras G12D expression p53 deletion (KP line) model To study antigen-specific responses, we engineered KP cells express antigen SIY certain experiments. CBT consisted combined anti-CTLA-4 anti-PD-L1 therapy. Results Orthotopic failed CBT, flank were controlled Lung contained activated providing platform compare functional in NSCLC. Single-cell RNA sequencing revealed that lacked effector exhaustion molecules despite clonal expansion. Analysis cancer-specific dysfunction was established during priming lung-draining mediastinal lymph nodes (mLN) robust proliferation. flow cytometry found primed mLN underwent blunted differentiation characterized lack CD25, Granzyme B, TIM-3, retention TCF-1. This phenotype persisted tumors, consistent our initial observations absent molecule expression. Many patients expressed analogous gene program distinct exhaustion. TCF-1+ did mediate tumor control differentiate into after investigate alternative therapeutic strategies reinvigorating tumor-reactive focused on IL-2 IL-12, their receptors reduced mLN-primed cells. Administering recombinant IL-12 sufficient restore differentiation, induce control, significantly extend survival tumor-bearing mice. Conclusions Our results suggest arise priming. Transient combination overcomes dysfunctional state protective tumors. Ethics Approval All mouse experiments approved MIT’s Committee Animal Care (CAC) - DHHS Welfare Assurance # D16-00078

Sentinel Node Mapping in Non-small Cell Lung Cancer Using an Intraoperative Radiotracer Technique
Objective(s): Lymph node metastases are the most significant prognostic factor in localized non-small cell lung cancer (NSCLC). Identification of the first nodal drainage site (sentinel node) may improve detection of metastatic nodes. Extended surgeries, such as lobectomy or pneumonectomy with lymph node dissection, are among the therapeutic options of higher acceptab...
متن کاملCancer/testis antigen expression and specific cytotoxic T lymphocyte responses in non small cell lung cancer.
Non small cell lung cancers (NSCLC) express cancer/testis antigens (CTA) genes and MAGE-A expression correlates with poor prognosis in squamous cell carcinomas. We addressed cytotoxic T lymphocytes (CTL) responses to HLA class I restricted CTA epitopes in TIL from NSCLC in an unselected group of 33 patients consecutively undergoing surgery. Expression of MAGE-A1, -A2, -A3, -A4, -A10, -A12 and N...
متن کاملMediastinal lymph node dissection in early-stage non-small cell lung cancer: totally thoracoscopic vs thoracotomy.
OBJECTIVES Although major pulmonary resections for early-stage non-small cell lung cancer (NSCLC) are more and more frequently performed through thoracoscopy, the adequacy of lymphadenectomy achieved via this approach is still questioned. The aim of this study was to evaluate the results of lymph node dissection (LND) during totally thoracoscopic (TT) major pulmonary resections. METHODS Clini...
متن کاملExtent of mediastinal lymph node dissection for clinical T1 non-small cell lung cancer.
The present study was designed to determine the extent of lymph node dissection for clinical T1 non-small cell lung cancer without negatively influencing curability. The study included 192 cases with clinical T1 non-small cell lung cancers who underwent lobectomy with mediastinal lymphadenectomy. Among 69 cases with right upper lobe tumors, metastasis was found in subcarinal lymph node in one c...
متن کاملRevised ESTS guidelines for preoperative mediastinal lymph node staging for non-small-cell lung cancer.
Accurate preoperative staging and restaging of mediastinal lymph nodes in patients with potentially resectable non-small-cell lung cancer (NSCLC) is of paramount importance. In 2007, the European Society of Thoracic Surgeons (ESTS) published an algorithm on preoperative mediastinal staging integrating imaging, endoscopic and surgical techniques. In 2009, the International Association for the St...
